How Patients Provide Consent

Patients who schedule an appointment with Core Pelvic Health & Wellness receive electronic intake forms through Embodia.

During completion of these intake forms, patients may voluntarily opt in to receive text messages by selecting a separate, optional SMS consent checkbox.

Consent to receive text messages is not required to receive treatment or any other services from Core Pelvic Health & Wellness.

Text messaging consent is separate from all other forms of consent, including treatment consent, financial policies, and intake documentation.
